____/____/____, and Turning Point Recovery Residence.
Resident commits to staying
at Turning Point until completing all twelve steps
of AA/NA with an AA/NA Sponsor from the directions
in either of the books of Alcoholics Anonymous or
NA Basic Text. Although the minimum time allowed
for completion of the steps is ninety days the actual
length of stay is based on my performance. Resident

be tentatively approved two weeks in advance by
the Executive Director and are contingent on all
rules being followed for the remainder of time in
the house.

does not cover all costs of the program and that,
as a nonprofit organization, Turning Point seeks
other funding sources to help defray operational
costs. Although the actual cost is $40 per night,
other funding sources pay $20 per night and the
consumer will be only be responsible for paying
the balance of $20 per night. 20 is for the bed
which is paid from other funding sources for qualifying
residents and $20.00 is for the administrative costs
to be billed to the resident once per week in advance
at $140. A $300 admission fee is due upon admission.
Admission fee covers the first week administrative
costs, a $20 miscellaneous non-refundable deposit
plus $140 refundable deposit if resident leaves
compliant. The administrative fee includes phone,
washer & dryer, kitchen use, cable, utilities,
salaries, office supplies, and other operating costs.
Linen, transportation, food, and household items
are not provided, however, bed linens are available
for those in need.
If
either party terminates the contract for any reason
at any time, there are no refunds and Turning Point
reserves the right to collect the unpaid balance
on 90 days. Upon any default in payment, resident
agrees to pay a reasonable attorney's fee for any
and all services of an attorney, whether in or out
of court, and for appeal and post-judgment collection
legal services.
Resident agrees to hold Turning Point harmless for
any type of injury or death resulting in any way
from his/her stay at Turning Point including riding
in vehicles. Resident waives all rights to his/her
assigns and successors of any legal action against
Turning Point. Personal safety is the resident’s
responsibility. Resident is also responsible for
any additional cost that may be incurred on resident’s
behalf such as charges for medical treatment or
prescribed medication. Resident is responsible for
getting doctor appointments and prescription medication
approved by the director in advance and turning
medication over to the manager to be dispensed by
the manager. Resident is responsible for asking
the manager for 24-hour doses of non-mood altering
drugs each day. Mood altering drugs, if approved,
will be dispensed in individual doses as needed.
Turning Point is not responsible for lost or stolen
property regardless of what anyone says.
Resident
has read and understands the House Rules. Resident
has received a copy and agrees to abide by said
rules. Failure to abide by House Rules may result
in immediate termination from Turning Point.
